**JOB SUMMARY**

Responsible to coordinate and integrate

evidence-based health promotion programming in group settings to augment care

coordination efforts with an emphasis on geriatrics.

**POSITION RESPONSIBILITIES**

Coordinates referrals, enrollments and schedules

for appropriate population. Expected to continuously develop, monitor, and

evaluate all evidence-based programming for quality improvement opportunities.

Coordinates and facilitates community education and events. Supports

organization-sponsored events. Role will include: coordination of classes,

guidance for lay leaders as a master trainer, assisting and or teaching EBP

programs, and work with community leaders and care team to build complementary

workflows. Expected to build strong alliances with community partners. Must be

able to facilitate group process, utilize adult learning principles, use

various teaching/learning methodologies and provide role modeling and

leadership. Within the education role, individuals may assist with the

administrative duties associated with program operations.

Coaches one or several individuals or teams on a specific self-efficacy subject area.

Contributes significantly to the well-being of team members and the larger community.

Develops a culture in which all members work constructively to enhance understanding of self-efficacy and confidence building to strengthen chronic disease self-management efforts.

Develops and monitors programs that integrate learning with practice opportunities.

Establishes and maintains productive working relationships within and outside own area.

Helps design creative strategies to reach care team members and EBP program participants for specific training courses.

Maintains current certifications in specialty and continues education when necessary.

Presents a professional image when interacting with others.

Promotes organizational learning through knowledge management, training and development, and sharing key experiences.

Seeks opportunities to help others learn as teacher, coach and mentor.

Selects learning activities that are related to specific competencies targeted for improvement.

Uses multiple presentation tools and techniques, adapting/fitting them to audience and topic.

**QUALIFICATIONS**

Bachelor's degree in nursing is preferred.

Graduate from a nationally accredited nursing program required, including, but

not limited to, Commission on Collegiate Nursing Education (CCNE),

Accreditation Commission for Education in Nursing (ACEN), and National League

for Nursing Commission for Nursing Education Accreditation (NLN CNEA).

Two years clinical experience in geriatric

settings required.

Currently holds an

unencumbered RN license with the State Board of Nursing where the practice of

nursing is occurring and/or possess multistate licensure if in a Nurse

Licensure Compact (NLC) state.
