Licensed Practical Nurse - VNA Health at Home Louisville

VNA Health at Home is seeking an experienced Licensed Practical Nurse for the Louisville area. The LPN is responsible for the provision of quality home services based on approved treatment plans. Must be able to observe and report abnormal occurrences or changes in clients and report to supervisor and document care provided. Must have a minimum of one year of nursing experience in a hospital setting. This is a full time 40 hour week position.

Responsibilities

• Provides care and education to patients and their caregivers.
• Participates in the coordination of services with other disciplines and community resources as needed.
• Performs visits as prescribed in the physician's plan of treatment and in compliance with state and federal guidelines.
• Participates in regular case conferences and in-services, and/or special case orientation.

Qualifications

• Graduate of an accredited school of nursing
• Currently licensed to practice in Kentucky
• One year of nursing experience required; one year of hospital nursing preferred.
