We are currently seek a Substance Abuse/Mental Health Counselor for our ACT team in Maricopa County, based out of Avondale.

Community Bridges, Inc. is one of the largest providers of integrated substance abuse/behavioral health treatment in Arizona. Recently our FACT model won the Most Innovative Best Practice through the Center for Medicaid Innovation. Our FACT teams provide community based services to individuals who have been diagnosed with a Severe mental Illness and are at risk for re-incarceration in the criminal justice system. We are currently seeking Substance Abuse/Mental Health Counselors for our FACT teams. In addition to the substance abuse/mental health counselors, the team is comprised of a psychiatrist, nurses, and various specialist positions (peer support, housing, employment, independent living skills). The goal of the team to provide intensive and specialized services to our members in the community to reduce hospitalization, reduce homelessness, reduce incarcerations, increase employment, and increase their ability to manage their healthcare. The teams are based out of central Phoenix and provide services throughout Maricopa County.

The Substance Abuse/Mental health Counselors on the team are responsible for providing weekly individual and group counseling to members on the team who are diagnosed with a co-occurring disorder. In addition, the substance abuse/mental health counselor will complete assessments, diagnosis, treatment plans, family counseling, psycho-education, case management and other services as applicable. Services may occur in-office, in-home or in-community.

Minimum Requirements:

  • Candidates must hold a master's degree or higher AND a current associate or independent level of licensure with the Arizona Board of Behavioral Health Examiners (AZBBHE) in the discipline of counseling, social work, and/or marriage and family counseling.
  • New graduates with a master's degree in counseling or social work who have recently applied for their license (LAC or LMSW) are encouraged to apply. Must have LAC, LMSW LAMFT, LPC or LMFT LCSW within 90 days of employment.
  • Must have valid AZ driver's license and clear 39 month clear MVR.
