Job Description

Christiana Care Health System (CCHS) is looking for APC Applicants (Nurse Practitioners/Physician Assistants) who have been licensed less than 18 months to join our innovative and customized 9 month APC Fellowship in Primary Care. This is an intensive program for new graduate NPs/PAs which provides mentoring by experienced NPs/PAs, specialty rotations, and additional didactic education in patient care in the primary care setting. This fellowship program is both supportive and challenging as APCs are seeing patients to engage in the clinical experience, while continuing to learn and grow through a structured didactic curriculum. This program is perfect for an APC who is looking to develop into the role of a primary care provider in a structured environment, with a Fellowship Coordinator providing guidance throughout the program. The APC Fellowship is based in a primary care practice with Fellows being given the opportunity to gain deeper exposure to education in specialty areas uniquely selected for each individual based on their experience clinical.

Each selected Fellow will receive an annual salary of $73,000. Fellows are considered full-time employees of Christiana Care with benefits. The expectation is that Fellows who meet performance objectives and demonstrate CCHS values will apply for permanent employment in a CCHS practice after completing the program at a rate commensurate with 1 year of experience clinical.

Principal Duties and Responsibilities:
Assess and document patient and family response to interventions.
Demonstrate ability to carry out critical skills as outlined in procedures and specific standards.
Identify each patient prior to administering medications and providing care and treatment.
Perform technical skills and administer medications in a safe and efficient manner.
Establish priorities based on patient needs and emergent care.
Provide supervision and direction to other team members. Delegate tasks according to capabilities and scope of responsibilities.
Ability to independently review patient problems including, but not limited to, medication refills, need of follow up care, and referring appropriately to the physician according to policy.
Perform all prescribed laboratory tests, send to appropriate laboratory, receive and interpret laboratory values, and notify physician of results.
Provide patient education and counseling to patients and their families.
Participate in chronic disease management of patients.
Provide diabetic disease management to patients.
Perform assigned work safely, adhering to established departmental safety rules and practices. Report to the appropriate personnel, in a timely fashion, any unsafe activities, hazards, or safety violations that may cause injury to oneself, other employees, patients, and visitors.
Demonstrate skills and knowledge necessary to provide care appropriate to neonatal, pediatric, adolescent, adult and geriatric patients, including knowledge of growth and development, and ability to obtain and interpret information to identify patient needs, and provide the care needed.
Perform other responsibilities as assigned.

Education and Experience Requirements:
Master's degree as an Advance Practice Registered Nurse -NP Licensure or license eligible as an Advanced Practice Registered Nurse - FNP as set forth by the state of Delaware
Prescriptive authority required in Delaware
Certification in BLS
Graduation from an ARC-PA accredited Physician Assistant Program
NCCPA Certification
DE State License and Prescriptive authority
